1. Cut the form to five fields or fewer

Every extra field you add increases drop-off. Research on web forms consistently shows that going from eleven fields to four can more than double completion rates. For a discovery-call form, you genuinely only need a handful of signals to decide whether a lead is worth your hour:

  • Name
  • Business email
  • What they're trying to solve (one sentence, open text — not a dropdown)
  • How soon they want to move
  • How they heard about you

That's it. Everything else — team size, tech stack, budget range — can come out in the call itself. Collecting it upfront just creates friction before you've given the person a reason to trust you.

2. Stop asking for budget before you've delivered any value

3. Respond within five minutes or lose the lead

A completed intake form is not a ticket in a queue. The person who just filled it out is warm right now. Studies on lead response time show that the odds of qualifying a lead drop by over 80% if you wait longer than five minutes to follow up.

Most AI agencies send an automated "Thanks, we'll be in touch within 2 business days" email and call it good. Your competitors do the same. 4. Use a conversational format instead of a static form
